Safety Alert – Brake Failure
A service truck was reported to have experienced brake failure while being driven down a slope at a mine site. The vehicle veered off the road and came to rest following contact with the safety berm. No injury was sustained by the lone driver in the High Potential Incident (HPI).
INITIAL OBSERVATIONS:
